Support For Clients With Developmental Disabilities

CHES provides comprehensive support for Maryland residents who have Spina Bifida, Autism, Cerebral Palsy, or other developmental disabilities and are living at home.

Particularly, its self-directed nursing support is suitable for those who are able and wish to maintain some degree of independence and control over their care plans. CHES can work with you to determine the most appropriate balance of self-direction and nursing support for your unique situation.

The organization offers two options for its self-directed nursing support services: Nursing Consultation and Nursing Case Management with Delegation.

Nursing Consultation

Nursing Consultation is best for clients with an HRST level of 3 or higher who manage their own healthcare and administer their own medications and treatments.

This option is also appropriate if you direct and take responsibility for your paid staff to complete your care tasks, or if you rely on unpaid caregivers, such as family members.

With Nursing Consultation, you'll also get support in developing your healthcare protocols and routines and effective methods for communicating with doctors.

Nursing Case Management & Delegation

Nursing Case Management and Delegation is best for clients who require more attention and support than Nursing Consultation tends to provide.

With this option, a CHES RN will work with you to develop your care plan and will then train your staff to carry the plan out day-to-day.

The RN will also delegate and supervise your staff to ensure your care plan is sufficiently carried out and you get everything you need to thrive. Your RN is also "on call" during business hours and will visit your home at least once every 45 days.

Getting Started With CHES

CHES uses each patient's Health Risk Screening Tool (HRST) level, their demonstrated degree of independence, medication needs, and current caregiver situation to determine which type of support service is most suitable.

To start the process of designing your care plan, a Registered Nurse (RN) from CHES will complete an HRST rating or review session with you, your family, and your care staff.
